After 35 years in the private practice of medicine, I am writing to inform you that I will be retiring December 30, 2014. Until that date, I will be available to serve your medical needs. It has been my pleasure providing for your health care over the years.
You will want to make arrangements to select a new ENT/Facial Plastic physician to make sure you receive uninterrupted medical care. I suggest that you contact your primary care doctor and/or your insurance to obtain names of ENT physicians that are in your plan. Another option is to call the University of Cincinnati ENT Department at (513) 475-8400. They have a large number of physicians that can treat all aspects of ENT and Facial Plastics.
Your medical records are confidential. If you would want a copy forwarded to you or to your new physician, you will need to complete an authorization to release these records. Unfortunately, your medical records cannot be released without a written authorization signed by you. For your convenience, an authorization form is available by clicking here. This form will need to be completed and returned to the office once you have selected a new physician. There will be a reasonable copying/mailing fee based on Ohio law to cover the cost of duplication and postage. Please allow at least 14 days to process your request.
